Icons are usually created either with dedicated icon making program, or simply by a graphics editor and renaming of the file extension. Of course PSD format is obvious choice for Photoshop, but this might also be doable with some alternative graphics editor that support Photoshop's formats. Also export to JPG/PNG might be needed as not every graphics editor support ICO format, which will require another tool for making the icons.
